Participation in the Survey of People with Diabetes Sweepstakes (the “Sweepstakes”) constitutes your agreement to be bound by these Official Rules.

Eligibility: Open only to legal residents of the United States, 18 years of age or older at the time of entry. Employees, officers, and directors of Sponsor, its subsidiaries and its advertising and promotion agencies, and their immediate family members and persons living in their same household, are not eligible to participate in the Sweepstakes. Void outside of the 50 United States, Puerto Rico and where prohibited by law.

Entry: You may enter the Sweepstakes by one of the following methods:

Internet: Click on the link at www.EngagingDiabetes.com, complete the survey and fill out the Official Entry Form by entering your name, and e-mail address and click the “Submit Entry” button. An email address is required to enter on-line.

Mail: You may enter without completing the survey on-line by sending your name, address, daytime and evening phone numbers, and e-mail address (if you have one) in your own handwriting on a 3” by 5” index card to: Survey of People with Diabetes Sweepstakes, 84 Eagle Street, San Francisco, CA 94114. Mail entries must be enclosed in a 4” by 9½” envelope with your name and return address placed on the front left-hand corner of envelope. Only one entry per envelope.

All entries become the property of Sponsor and will not be returned. No mechanically reproduced entries will be accepted. Only one entry per person is permitted.

Sweepstakes Period: The Sweepstakes begins on March 1, 2009 at 12:00 a.m. Pacific Time and ends on May 1 2009 at 11:59 p.m. Pacific Time. All mail entries must be postmarked by May 1, 2009 and be received by May 8, 2009.

Prize: The winners will receive a $30 Amazon.com gift certificate with a retail value of $30. Ten (10) prizes are available. All prizes will be awarded. No alternative prize, cash or other substitution is permitted except in the sole discretion of the Sponsor in the event of prize unavailability. All federal, state and/or local taxes are the sole responsibility of the winner.

Selection of Winner/Odds of Winning: The winner will be selected from all eligible entries received in a random drawing on or about May 15, 2009. Winner will be notified by phone or e-mail and by U.S. postal mail. If a prize notification or prize is returned as undeliverable, the prize will be forfeited and will be awarded to an alternate winner. As a condition to receiving the prize, the winner will be required to sign and return a affidavit of eligibility and publicity release. The winner must agree to the use of his or her name, voice and likeness for promotional purposes without further compensation, unless prohibited by law. Winner should allow approximately 3 to 4 weeks after mailing the affidavit of eligibility and publicity release to receive the prize. The odds of winning the prize depends on the number of eligible entries received.

General Conditions: No purchase necessary to enter or win. Sponsor is not responsible for and shall not be liable for: (i) misdirected or delayed mail or e-mail; (ii) telephone, telecommunications, hardware or third party software malfunctions, failures, or difficulties of any kind; (iii) any condition caused by events beyond Sponsor’s control that may cause the Sweepstakes to be disrupted, corrupted or terminated; (iv) any injuries, losses, or damages of any kind arising in connection with or as a result of participating in the Sweepstakes or possession of the prize; or (iv) any printing or typographical errors in any materials associated with the Sweepstakes. Sponsor reserves the right to disqualify, in its sole discretion, any person tampering with the entry process, the operation of the systems required to conduct the Sweepstakes or operate the Sponsor web site, or who is otherwise in violation of these Official Rules. All issues concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of the participants, shall be determined by Sponsor in its sole discretion and governed by California law.

Requests must be received no later than July 1, 2009.
